When do you turn a maintenance contract down?

When we cannot answer for what is there. The most repeated cases: equipment with no support and no spares to be had; installations with no administrator passwords and no way of recovering them; installations that breach something the client does not want to correct; and places where we are not guaranteed access when we need to get in.

And one more, which is one of the honest ones: when what is being looked for is the price of a maintenance contract that can only be given by not making the visits. That contract exists in the market and we would rather not sign it.

In all those cases the report explains what would have to be done for the installation to be maintainable. That is just as useful to the client even if somebody else ends up doing the work.
